Job Summary / Purpose:
This project supports the urgent need to expand intake and placement operations for families transitioning out of the Row and into the shelter system. In alignment with the Agency's goal of completing all transitions by January, operations must now accommodate up to 60 families per day and operate over extended hours to meet the accelerated timeline.
Support a smooth and efficient transition process – these specialists will provide direct support to families during their relocation, helping to assess needs, gather documentation and identify appropriate shelter placements based on family composition, school enrolment and medical or other considerations. They will coordinate closely with other Client teams and contracted providers to ensure families are transitioned safely, efficiently and with minimal disruption. Outreach specialists will also need to maintain accurate records, assist with case updates, and help monitor the process of transfers. Skills needed include strong communication and interpersonal skills, ability to collaborate effectively with various groups of individuals and manage sensitive information with discretion, flexibility to work under tight deadlines and adapt to a fast-paced environment.
This project is critical to meeting operational goals while maintaining the dignity and stability of families during their transition.
